What aged and disabled waiver looks like
Every visit is built around your loved one, but here are some of the everyday moments families can expect.
- 1Step 1: Financial eligibility. Apply for Medicaid through iServe Nebraska, by calling DHHS at 1 (855) 632 7633, or by emailing dhhs.economicassistance@nebraska.gov.
- 2Step 2: Level of Care (LOC) assessment. Once Medicaid is approved, request an assessment through your local Area Agency on Aging. In the Omaha area this is the Eastern Nebraska Office on Aging (ENOA).
- 3Step 3: A service coordinator will be assigned to you from your local area office on aging.
- 4Step 4: Care begins. Your Services Coordinator sets up approved services such as personal care, medication reminders, homemaker help, and respite with a provider like Hope Care Services.
