    What is Immanuel Pathways (PACE®)?

    Immanuel Pathways is a Program of All Inclusive Care for the Elderly, known as PACE®. It is both a comprehensive healthcare program and a health plan designed for older adults who need a nursing home level of care but want to stay safely at home. Instead of juggling a fragmented healthcare system, PACE® participants get customized care from a dedicated interdisciplinary Care Team that coordinates everything from primary care and home support to transportation and social services.

    Every visit is built around your loved one, but here are some of the everyday moments families can expect.

    On site primary care, therapy, and adult day activities at the Immanuel Pathways Center.
    In home skilled nursing, personal care, light housekeeping, meal prep, and grocery shopping.
    Specialty care including audiology, dentistry, optometry, podiatry, cardiology, neurology, and psychiatry.
    All medically necessary prescription and over the counter medications.
    Durable medical equipment such as wheelchairs, walkers, and oxygen.
    Care coordination for hospital stays, emergency room visits, and skilled rehab.
    Nutritional counseling, social services, and recreational therapy.
